Why fluent is not the same as correct

A generated answer can be grammatically confident and factually wrong at the same time; fluency and accuracy are produced by different mechanisms.

A language model predicts plausible continuations of text. It has no built-in mechanism that halts generation because a claim is false; a wrong answer reads exactly as smoothly as a right one.

Guard against this by separating retrieval from generation, requiring an exact source citation for every claim, and treating an unsupported statement as a defect regardless of how well it reads.
