Accountability does not transfer with automation

Automating a step does not automate the responsibility for its outcome; the named owner remains accountable for what the tool produces under their process.

When a tool contributes to a decision, the record should show who reviewed the output, what evidence they saw, and what they decided—not just what the tool returned.

This is why acceptance and override logging matter: they preserve the chain of accountability that regulators, customers and auditors expect to see.
