Handling an incident
A missed finding or a false finding discovered in production is an incident, not a footnote; it should be logged, investigated and used to decide whether the capability continues, is restricted, or stops.
Treat an incident the way a quality system treats a non-conformity: contain it, understand its cause, correct the immediate case and decide whether systemic action is needed.
A pattern of incidents that keeps getting explained away individually is itself a signal the control did not survive contact with real production evidence.

- 01Log every incident with enough detail to investigate.
- 02Decide continue/restrict/stop explicitly after each incident review.
- 03Watch for a pattern across incidents, not just each one in isolation.
