Which high-severity failure mode has no prevention control?

Explore how requirements, characteristics, controls, measurements, findings and field learning connect. Solid means confirmed; dashed means inferred; broken means a gap; stale means revision mismatch. Think of it like a home's wiring diagram: each node is an outlet or switch, and a "broken" edge is a wire that's supposed to connect two points but doesn't — nothing will trip until someone needs that connection.

Example: the failure mode "uneven tower flange contact" (F-08) should connect to a prevention control — a machining inspection plan, say. If that link shows "broken" below, it means quality has identified a known way for this to go wrong, but nothing is currently in place to catch it before it happens. That is the gap worth finding before an audit does.

WHY THIS INSTRUMENT EXISTS

A critical characteristic on a drawing should be traceable through risk analysis, control plan, work instruction and inspection evidence — and in most organisations it is not, because those artefacts are maintained separately and drift apart silently between revisions.

Connecting evidence by revision, part, project, supplier and phase turns a set of disconnected documents into a graph a reviewer can actually walk end to end, which is where the highest-value findings usually sit — between documents, not inside one of them.
